TRINITY PATH WELLNESS

PRIVATE NURSE NAVIGATION PATIENT ADVOCACY WELLNESS SUPPORT
Helping you navigate your health with greater clarity and confidence.
Healthcare and life can become overwhelming. Whether you’re navigating a new diagnosis, recovering after hospitalization or surgery, caring for an aging parent, trying to understand a complicated care plan, or simply feeling overwhelmed and ready to feel better, you don’t have to figure it all out alone.
I provide personalized nurse navigation, patient advocacy, education, and wellness support to help you understand what’s happening, organize your next steps, and move forward with greater confidence.
Not sure what kind of support you need? That’s okay. We can begin there.
How I Can Support You
Private Nurse Navigation &
Patient Advocacy
When healthcare becomes complicated, I help you make sense of what comes next. Support may include hospital-to-home transitions, understanding and organizing existing care plans and discharge instructions, medication organization and education, preparing for medical appointments, surgical preparation and recovery, aging-parent and family support, navigating rehabilitation and home health resources, and helping you identify important questions for your healthcare team. My goal is to help you feel more informed, prepared, and confident as you navigate the healthcare system.
WELLNESS STRATEGY & SUPPORT
Sometimes you are not facing a medical crisis, but you know something needs to change. Together, we can explore nervous system regulation, stress and overwhelm, sleep, lifestyle foundations, boundaries, sustainable habits, and patterns that may be affecting your overall well-being. This is a supportive space to slow things down, better understand what may be contributing to how you feel, and develop realistic strategies that support meaningful and sustainable change. ONGOING NAVIGATION & SUPPORT
Some situations require more than one conversation. Ongoing support is available for individuals and families navigating changing health circumstances, multiple healthcare providers, recovery, caregiving responsibilities, or periods of significant stress and transition. Having consistent support can provide continuity as circumstances change, helping you organize information, prepare questions, understand the next steps, and feel less overwhelmed along the way.
NOT SURE WHAT KIND OF SUPPORT YOU NEED?
That’s okay. You don’t need to know whether you need nurse navigation, patient advocacy, wellness support, or a combination of all three. We can begin with a consultation, talk about what’s happening in your life right now, and determine together what type of support would be most helpful.
MEET KELLY KRISTINE, RN
Private Nurse Navigator • Patient Advocate • Wellness Strategist • Founder, Trinity Path Wellness
I’m Kelly Kristine, a Registered Nurse with nearly 30 years of healthcare experience and the founder of Trinity Path Wellness. Throughout my career, I’ve had the privilege of caring for people and families during some of life’s most vulnerable and uncertain moments. I’ve also seen how overwhelming healthcare can become when you’re trying to understand a diagnosis, navigate a changing care plan, care for someone you love, or simply figure out what comes next. Those experiences helped shape the work I do today. Through Trinity Path Wellness, I combine nursing experience, patient advocacy, education, and whole-person wellness support to help individuals and families feel more informed, prepared, and confident as they navigate their health and well-being.I believe good support isn’t about telling people what to do. It’s about listening, helping them understand their options, asking thoughtful questions, and empowering them to become more confident participants in their own health and well-being.
Sometimes you need information.
Sometimes you need an advocate.
And sometimes you simply need someone knowledgeable to listen and help you find your way forward.
MY PHILOSOPHY
I believe meaningful change begins with awareness. Health is rarely shaped by one thing alone. Our physical health, emotional well-being, nervous system, sleep, nutrition, movement, relationships, environment, daily habits, and life experiences are deeply interconnected. Nearly three decades in healthcare have also taught me that people need more than information. They need to feel heard, understand what is happening, know what questions to ask, and feel empowered to participate in their own health and well-being. Whether I’m helping someone navigate a complicated healthcare journey or supporting someone who simply wants to feel better, my approach is the same: meet the person where they are, look at the whole picture, and identify realistic next steps forward. I don’t believe in perfection or one-size-fits-all solutions. I believe in education, self-awareness, thoughtful guidance, and sustainable change. Sometimes healing begins with changing what we do. Sometimes it begins with understanding what we need. And sometimes it simply begins with becoming aware of what is no longer working.
✨️ Awareness is where healing begins.
YOU MAY BE IN THE RIGHT PLACE IF…
You’re trying to make sense of a new diagnosis, changing care plan, or complicated healthcare situation.
You or someone you love is coming home after a hospitalization, surgery, or rehabilitation stay, and you’re unsure what comes next.
You’re caring for an aging parent or loved one and feel overwhelmed trying to coordinate appointments, medications, providers, and next steps.
You have questions about your healthcare but aren’t always sure what to ask or how to communicate your concerns to your healthcare team.
You’re preparing for surgery or recovering afterward and would feel more confident having an experienced nurse help you understand and navigate the process.
You’re managing multiple providers, appointments, instructions, or medications and need help organizing the information and seeing the bigger picture.
Or maybe you’re not navigating a medical crisis at all. You may simply feel overwhelmed, stressed, depleted, or disconnected from your own well-being and want knowledgeable, compassionate support as you work toward greater balance and sustainable change.
Whatever brings you here, you don’t have to have everything figured out before reaching out. We can begin with where you are.
WHY WORK WITH ME?
For nearly three decades, I’ve cared for individuals and families across a wide range of healthcare settings. That experience has taught me not only how healthcare works, but also how overwhelming it can feel when you’re the person trying to navigate it.
A Nurse’s Perspective
I bring a clinical nursing perspective to our conversations while helping you better understand information, organize your questions, and communicate more confidently with your healthcare team.
Personalized, One-on-One Support
You are more than a diagnosis, a chart, or a checklist. Our work begins with listening to what’s happening in your life, understanding what matters most to you, and determining what kind of support would actually be helpful.
Whole-Person Approach
Healthcare navigation and personal well-being don’t exist in separate worlds. Stress, sleep, relationships, lifestyle, nervous system regulation, caregiving, and life circumstances can all influence how we experience our health.
Independent Support Centered On You
My role is to support, educate, advocate, and help you navigate. I don’t replace your physicians or other healthcare providers. I help you become a more informed, prepared, and confident participant in your own care and well-being.
LESSONS FROM THE BEDSIDE
Reflections from nearly 30 years in nursing on health, healing, and the human experience. Some of the greatest lessons I’ve learned were not found in textbooks. They were learned at the bedside. Throughout my nursing career, I’ve had the privilege of caring for people during some of the most vulnerable, difficult, hopeful, and transformative moments of their lives. Those experiences have shaped not only the nurse I am, but also the way I understand health, healing, resilience, and what people truly need when life becomes difficult. Lessons From the Bedside is where I share those stories, observations, and reflections, along with the wisdom that nearly three decades of caring for others has taught me about being human.
YOU DON’T HAVE TO NAVIGATE IT ALONE
Whether you’re facing a complicated healthcare situation, caring for someone you love, trying to understand what comes next, or simply feeling overwhelmed and ready for greater clarity and support, you don’t have to figure everything out on your own. We’ll begin with where you are, talk about what you’re navigating, and determine what kind of support would be most helpful for you. Not sure where to begin? That’s okay. Sometimes the first step is simply having a conversation.
